SCENARIO:
Your client has been on bed rest at home but was not being very compliant. She was admitted to the unit as the following: G3 P2, at 38 weeks in active labor with the vaginal exam, 4cm, 75%, -1, and intact membranes. The following assessment was completed:

1). Urine protein: trace
2). Headache times 2 days, rated at 5 out of 10 on the pain scale
3). Visual disturbances: blurry vision noted with occasional “floaters”
4). Epigastric pain: negative
5). Slight swelling of hands, feet, and face
6). Nausea and vomiting: Denies
7). Checked reflexes and clonus; 2+ with no clonus
8). Blood pressure= Initial B/P with results of 147/93. Several serial B/Ps were gotten after changing to a larger cuff: 145/89, 140/85, and 146/92.

After notifying the client’s physician, what labs would you expect the physician to order for your client, and for what reason would these be obtained?

All labs came back normal. The client progresses with labor, has an epidural started, and vital signs remain stable. She delivers a healthy 8lbs- 6 oz boy six hours after admission. The client was transferred after a two-hour recovery to the postpartum unit with her baby.

On her first day postpartum, the client is complaining of a splitting H/A. As the nurse caring for this client, what questions would you ask the patient? What would be part of your nursing assessment? Include critical thinking and the nurse’s role in caring for this client.
